Start Here: Find Your Assessment Documents
Most disciplines will begin by reviewing their current assessment documents for the AY25–AY29 cycle
Assessment Document Tables
- AY25 – AY29 Assessment Document Table
Use this table to access:- Discipline Assessment Cycle Schedule (DACS) – Shows when planning, data collection, and reporting occur across the 5-year cycle. If the Courses/Description field is blank, the schedule is incomplete.
- Discipline Assessment Activity Report (DAAR) – Formerly the Discipline Assessment Project Plan. Includes mapped course competencies and program learning outcomes, identifies key assignments for HCA and Canvas data collection, and serves as the final report for findings and future plans.

Template Requests
- Request a new Assessment Activity Report Template
Use this template to document your assessment activity details, including what will be assessed, the timeline for data collection, and reporting after data collection is complete. Please align your activity with the cycle schedule.
